LEAP /GEE and iLEAP TESTS
REGISTRATION FORM -2008-2009

     A student who is transferring from an in-state nonpublic school or a home schooling program or a * Louisiana resident who is transferring from an out-of-state school to enroll in the Louisiana public school system at grades 5 or 9 shall be required to take the 4th or 8th grade LEAP  English Language Arts AND Mathematics tests.  Required achievement levels:

   LEAP  - 4th and 8th  Grades- A minimal of BASIC on ELA or math and  APPROACHING BASIC  on the other.

    All nonpublic and home school students who wish to take the LEAP Tests (Grades 4 and 8); LEAP GEE Tests (Grades 10 or 11); or iLEAP Tests (Grades 3, 5, 6, 7, and 9) must complete the following registration form and mail it to the address below.

NOTE:  iLEAP IS NOT ADMINISTERED DURING SUMMER TESTING.

Students will be tested at the grade-appropriate school within their district.

(If your child attends a nonpublic school that has chosen to register the students through the school, the scores will be released to the school unless the District Test Coordinator is notified by the parent in writing  to NOT release the scores to the school.)
