Student Participation Permission Form

My child _______________________ has my permission to attend LMS Christian Student Union club meetings each Wednesday morning between 7:30 and 7:50.  The club meets on the Loranger Middle School campus.  I understand that students attending this club will pray and read from the Bible with other students.  This club is completely student initiated, but there will be an adult supervising the club meetings.
